JavaScript for validation failing for EPiServer.Forms

Im receiving an error with the javascript for validation. I have followed this thread (http://world.episerver.com/forum/developer-forum/Addons/Thread-Container/2016/4/enable-the-client-side-validations-using-episerver.forms/?pageIndex=1#reply) and I have made no progress. Here's the error:

http://domain:17000/EPiServer.Forms/DataSubmit/GetFormInitScript?formGuid=a0000aa0-b11b-22c2-3333-4e44ee44e444&formLanguage=en

500 (Internal Server Error)

What am I doing wrong?

I've been able to resolve the 500 error and any other errors that were occurring in the referenced thread but thre is still no client-side validation. Please help!

This is what I'm seeing: This is the Server Side validation error. Nothing Client-Side.

I am no longer receiving any errors I just have no server side validation at all.
