Setting TAX but NOT for countries

Hi,
Is there a way to set the tax to work regardless of Country code? We have a client who has exam courses that run in the UK and across the world and people regardless of address can book and attend the exam. The TAX is only set at per line item level regardless of where the person is living.
So for example, an EXAM in the UK has a TAX value of 20% and a person living in SPAIN wants to but this item - he will still need to pay 20%.
A different EXAM maybe TAX excempt so there is NO TAX on this item so the person from SPAIN can also buy this exam too.
Plus, many of our customers do not have country codes so we couldn't match these up with the current set up.
