Am I missing something? Optimizely forms bug fix

Is there any errors or console logs which are appearing for this?
Could be worth replicating the error by installing the Alloy template site locally and installing the version of Forms mentioned in the bug to test/verify if has been bug has resolved here to help pinpoint the issue.

If it still hasn't been fixed on the Alloy site, could be worth contacting Support to reopen this bug and fix where needed.

I am running through applying a range of updates, and going through all that have listed changes associated.
There is a change associated with Episerver.Forms 5.6.1: Release note - AFORM-3458 (optimizely.com)
However, when I place the config in appsettings, it does not insert the header for static files on form pages.
Am I just missing an associated change?
