I have tried altering the UIURL and UTILURL settings in the config to include the full url, but this has made no difference. This use to be possible is CMS5 R1 (before the ability to use relative paths for these settings (which is a good thing and has solved other problems I have had)) but doesn't seem to work anymore.

Is it possible to configure EPiServer to use a different URL for the UI and Util folders instead of the main site?
For example:
The website would be accessed via http://www.mydomain.com, but the only way to access the UI and Util folders would be http://www.myotherdomain.com.
