What is the best practices for patching Episerver? 

What is the best patching plan for on-premise Episerver environment?

Within minor versions it should be as straight forward as updating nuget packages.

Between major -might be more interesting.

More essential question might be - what is best practices to apply patch to production/staging/whatever environment. How to minimize downtime and do it quickly.
