Hi, this is a bug CMS-3486 with Forms 2.1 and CMS 9.8.3. I am sorry for this.
For work around, please install specific version of CMS 9.8.2.

<p>Perhaps the error is two-fold..</p>
<p>There is a problem CMS.Core 9.8.3 when you have a DisplayAttribute together with a validation attribute and where the Name property on the DisplayAttribute is not set. This is registered as <span>CMS-3485 and is hopefully released in the upcoming CMS.Core release (9.8.4 or 9.9.0) .</span></p>
<p>That version of CMS.Core should work with Forms 2.1.0.2</p>

I have upgraded the site from 9.3.1 to 9.8.3 and installed the EPiServer Forms 2.1.0.2. I have drag and drop Selection and Multiple or Single choice Form elements and when I try to add items i am getting this error "Unable to load /EPiServer/cms/Stores/contentdata/ status: 500"
