Forms: Include the name of the page in the SUMMARY placeholder

I would like the SUMMARY placeholder to include the name of the page the form was submitted on.

Also if possible the URL of the page.

Or at least have the page name as an option in the list of other placeholders to choose from.

If you're using Forms 4.3.0 and higher, you can implement your own IPlaceHolderProvider
