There is a solution for this that are beeing tested and will soon be released.
The problem is that today the extension can not handle when the dpi is other than 100%.
Check for updates on the extensions and updates in Visual studio

until you guys find a solution its possible to downgrade the dpi and afterwards create an episerver project.

The fix is in testing right now so hope to get it out within the next 2 weeks or so.
I'm recently experiencing problems creating EPiServer CMS projects in VS 2013 and 15.
In VS 13 and 15 I choose File > New Project > EPiServer Web Site > Ok
Afterwards I cant choose between empty, webform or MVC, however there is no ok button
