You need an EPiServer installation on each dev machine, pointing to the test server db.
What version of EPiServer 4 are you using? I am guessing somewhere along the lines of 4.51 as you are running VS 2003 (.NET 1.1?), but I might be wrong. Try the Tech notes/White papers-section of this web site, it will get you started: http://world.episerver.com/Documentation/Categories/Documentation-Type/Tech-notes/

Another question - on my local dev environment - can my Episerver site run from a Virtual Directory? Or does it have to be at the root?

I have the following scenario:
Episerver 4 installed on Test Server
Episerver DB installed (SQL Server 2005) on Test Server
Local instance of VS 2003 and source code
We have more than 2 developers working on the solution. Do we need an instance of Episerver running on each dev machine (pointing at the Episerver DB on the test server)? Or can we just point our local dev instances to Episerver on the Test server?
Any links to helpful articles would be useful?
Bags of .Net experience but none of Episerver!
