Creating a questionnaire page
Not sure what you mean by "customizable" in this case, but in "standard" EPiServer CMS (public templates) there is a specific page template available for creating Web forms. Using this page template editors can create their own forms with text boxes, radio buttons, drop-down choices etc. The functionality is all built into the page template.

Hi,
I would like to know if it is possible to create a customizable questionnaire page.
I would like to create a page with three questions and a text box under each for users to be able to respond.
It would also have 6 fields for name first name etc...
If it is not possible, could someone tell me what is the closest template I could use for that purpose ?
