Approve and Publish button fails to move pages out of the Ready to Publish state in EPiServer CMS 8.

We have an EPiServer 8 site where end users are using the "Ready to Publish" option in the Tasks tab.  We have some pages in the Ready to Publish state.  After selecting Publish > Approve and Publish, the green button animates to show "Publishing..." then the Options dialog disappears.  Beside the Options dialog we see the "No changes to publish message."  And opening the Options dropdown I see "Not changed since last published" and "Last published by You, Today..." followed by the current time.  And we can see that the change is published.

But the version doesn't disappear from the Tasks list, even if you click the Refresh button beside the Ready to Publish dialog, or refresh the page.  If you click on that document under the Tasks tab, then the "Ready to publish" yellow arrow appears pointing to the "Publish?" button.  You can expand and see the "Approve and Publish" button again.

The message in that dialog shows "Pending approval sent by you, Today 3:02 PM", followed by the "Approve and Publish" button, followed by a message, "Last published by You, Today 3:51 PM."  Something is causing those versions not to be removed from the Ready to Publish state.
