Can't save form data to database when using "Send custom email example"

Hi, The default xform example works well in EPiServer 4.61 ( .net 2.0 templates ), but when we using code example http://www.episerver.com/en/EPiServer_Knowledge_Center/developerforum/Code-Samples/6042/Send-custom-email/, the form data can not be saved into database ( We have inclulded ChannelOptions.Database to ChannelOptions ).
