Is it possible to add custom additional feild in the CSR UI? here Return Details, Comment is good, but we'd like an other comment field.

And how do you add a custom field to the shipment or order? what is preferable?

In theory, you can use the API (MetaDataPlus MetaField and MetaClass) to add new metafields to OrderForm. I recall that custom fields are displayed now, but I'm not sure. There are 2 caveats however:

There is currently no UI to administer metafield/metaclass as far as I know (only for Business Foundation).
Also that would be affecting the common OrderForm (Return Form is just another OrderForm).

Been a while since I touched this, I probably need to dive into it, when time permits.
