I installed the bvn 404 nuget package.  Can't seem to find the gadget.  Any assistance is appreciated.

It is likely that you are missing the configuration under protected modules:

<episerver.shell>
    <protectedModules>
        <add name="BVNetwork.404Handler" />
    </protectedModules>
</episerver.shell>
