Do you get any errors ni the log file? There, you should get an error message with more info on why the image didn't get uploaded. It could be that the image is too big, then you need to check the web.config values for the max upload size.
Do you also have any OnSaved or OnPublished events, perhaps some of them are failing due to a wrong cast or so?
In any case, log file should give you more info.

Which CMS version are you using? How do you upload the image and where/when do you get the error message? Can you also send the error message?
We just had an upgrade from CMS 6 R2 to 7.5. Looks like a permission issue.
Jen, that's a permission issue. Make sure your application pool identity user has write permission for c:\interpub\vpp for aab.relationhouse.com folder

The uplod files dialog says that blobs are stored in c:\interpub\vpp... but your screenshot is for c:\episerver\vpp\vpp...
I get a 'Failed' message when uploading an image in the Assets pane, although the application has content types that can hold media files. What could be wrong?
